• 1×1 Pictures white background in Matte Photo Paper (6 copies)

• Notarized Latest Certificate of Employment with Compensation (COEC)

• PSA Birth Certificate if Single with receipt (CENOMAR might be required)

• PSA Marriage Contact if Married with receipt

• Passport Photocopy of Entry and Exit stamp
